panfrost: Move MALI_EXTRACT_INDEX to pan_format.h

Move MALI_EXTRACT_INDEX to pan_format.h where all format-related macros
live and kill the unused MALI_EXTRACT_TYPE and MALI_FORMAT_COMPRESSED
macros.
