0c0819f0da aco/cssa: reset equal_anc_out if merging fails 
try_merge_merge_set() expects equal_anc_out to be Temp() in the beginning,
so we need to reset it in case it's used again.

Fixes compilation of metro_exodus/163b3b895730d37b with
VK_PIPELINE_CREATE_2_DISABLE_OPTIMIZATION_BIT_KHR.

bb956464cb aco: Skip code paths to emit copies when there are no copies. 
Found while running with libstdc++ debug mode.
Fixes the following:

Error: attempt to advance a dereferenceable (start-of-sequence)
iterator -1 steps, which falls outside its valid range.

18ba93e673 aco/cssa: rewrite lower_to_cssa pass 
The previous pass was based on misconceptions and
rounded up with bug fixes. The new pass is entirely
rewritten and basically just one-to-one from the paper:
 "Revisiting Out-of-SSA Translation for Correctness, CodeQuality, and Efficiency"
 by B. Boissinot et al.
It also incorporates the value-equality testing.

The regressions are mainly due to creating parallelcopies for
exec phis at loop headers (mitigated in the next commit).

Totals from 4933 (3.61% of 136546) affected shaders (Raven):

d2c18b7bf3 aco: use bit vectors for liveness sets 
This seems to be much faster than hash sets. When compiling pipelines from
5 games, live_var_analysis takes about a third the time it used to and
fossilize-replay is ~1.77% faster.

d098024c40 aco: rework lower_to_cssa() 
This patch changes lower_to_cssa to be much more conservative
about assumptions which phi operands might interfere.
Previously, this pass wasn't exhaustive and could miss some corner cases.

v2: remove optimizations to find better insertion points as it's hard
to guarantee that they are always correct and have overall no benefit.

0b8216b2cd aco: Lower to CSSA 
Converting to 'Conventional SSA Form' ensures correctness w.r.t. spilling of phi nodes.
Previously, it was possible that phi operands have intersecting live-ranges, and thus,
couldn't get spilled to the same spilling slot. For this reason, ACO tried to avoid to
spill phis, even if it was beneficial.
This patch implements a conversion pass which is currently only called if spilling is necessary.
